Pass Completion Rules

What do leagues or even as individuals call a completion?

Do you have to hit the intended receiver or it's base with the ball in the air or does anyone allow the ball to slide/skim the field surface first then hit the player or base and call that a completion?

Just wondering what most of you are playing like. I get a felling you are going to say the ball must hit the figure or it's base first without hitting the field.
