How do one pull off a counter or trap?

Default my take on the pulling guard...

Pulling guards- simply reverse the lineman base and create a strong arc so your lineman pulls back and around to lead the way thruy the intended hole. The divider style of play is very condusive to this since the defense can't see the setup of the O-line.

Lateral dials (ITZ, Munroe)- these can be used in the same way as stated above and are hidden "under the hood". The secret is to tweak the ITZ dial to run with an arc so when it pulls out backwards it can arc towards the intended direction to lead thru the hole.
